An employee was working on an airlock system. Rotating cups within the system lacerated the employee's left index finger; the fingertip was medically amputated.

On December 31, 2018, an employee was trying to break up a fight between two clients when a door was slammed on the employee's right ring finger. The fingertip was amputated.

An employee was cleaning plastic debris off a machine. The employee's left index fingertip was caught in a chain and sprocket, suffering a partial amputation.

An employee was cleaning out a compound receiving bin to remove a jam around the flow valve. When the compound receiving bin flow valve became dislodged, the compound bin started again and amputated the employee's left middle fingertip.

An employee was helping others tuck a new sheet onto a re-reeling machine spool. His right hand got caught in the spool, and the machine pinched his middle, ring, and little fingers. He was hospitalized and his little finger was surgically removed just above the nail.

An employee was unjamming a package on a conveyor and caught their hand in a pinch point resulting in the amputation of the left pinky finger.

On December 29, 2018, two employees were moving a drum with a forklift. One employee's hand was caught between the drum and the forks of the forklift, causing an amputation of the right middle finger.

A crew was removing and replacing shafts/rolls on a conveyor using an overhead crane. A roll did not fit correctly and needed to be repositioned. A wire rope choker was positioned in a basket configuration around the shaft/roll with the ends of the choker connected to the crane hook. The injured employee was holding the top legs of the choker as the operator lifted up on the hook to put tension on the choker. While the employee was holding the choker legs, the crane hook started to twist. His left hand and glove became tangled between the steel choker legs. His left index finger was stuck between the choker legs, resulting in a distal phalanx amputation.

An employee was handling a chain binder that was positioned between the gap of two deck plates inside the hold of a ship. The fourth distal phalange was pinched between the blade of a forklift and the chain binder that was being held causing an amputation injury.

An employee was walking/standing and waiting for instructions on the floor next to a machine, when his right hand became entangled by the fiber and the machine. The middle finger was amputated.

An employee was repairing a chain on a forklift when the forks suddenly moved causing the employee's hand to get caught in the chain. The employee sustained a fingertip amputation.

An employee was cleaning a motor/chain drive on an overhead trash line. The sprocket caught the rag he was using, pulling his right hand into the chain. His right little and ring fingers were amputated.

Two employees were on a rig, adjusting arms to line up the holes with the bracket of a power wrench. The bracket caught an employee's left index finger, causing a partial amputation.

An employee was lubricating a tabletop paper punch machine. The machine activated and caught and amputated the employee's right ring fingertip.

An employee was untangling the whip line and main line of a crawler crane. The employee finished untangling the crane lines and the crane began to move. The main line caught the employee's hand, causing an amputation to the left index finger past the cuticle, an amputation to the left middle finger halfway into the nail bed, and a fractured ring finger.

An employee was cleaning the rollers on a printing press with a rag when a finger was caught in the press causing an amputation injury.

An employee was checking the rollers on a conveyor when their glove was caught in the rollers and belt resulting in a fingertip amputation.

An employee opened a safe to make change. When the safe closed, it struck his right thumb and severed the tip. He was hospitalized, requiring plastic surgery.

An employee was assisting with the cleaning of a batter applicator machine. While removing a segment of poultry from a pipe, which was connected to a 1.5-inch positive displacement pump, the employee's right index fingertip was amputated by the pump.

On December 27, 2018, a security officer was struck by a motor vehicle while working at a base entrance gate. The employee suffered a crushed left leg, which had to be amputated.
