An employee was trying to remove a piece of stuck bark debris from a conveyor system. The conveyor caught the employee's left hand, lacerating the thumb and possibly amputating the index fingertip.

An employee was up in a lift, looking at a conveyor belt to find its serial number. The machine was turned on and the belt broke his left middle finger and removed the skin. He was hospitalized, undergoing surgery to amputate the fingertip.

An employee was working from a barge, stabilizing a pier. While the employee was removing dunnage, the employee's left middle and ring fingers were caught between an H-beam and a steel plate. The ring fingertip was surgically amputated.

An employee was using a winch and a cable when his hand was caught between the winch and a truck frame. The employee suffered an amputation.

The employee had the pry bar in the grate and was applying pressure to pop it out while reaching his right hand under the grate end to help lift it. The pry bar then slipped out and onto his right index finger, pinching it. He reacted by pulling his gloved hand out, which resulted in a right index fingertip amputation.

An employee was cutting wood for truss assembly using a radial arm saw. The employee had a seizure while operating the saw and the tip of his left thumb was amputated.

An employee was using a 24-volt battery-powered drill to drill into a plastic vault used for telecommunication equipment when the glove on his left hand was snagged by the rotating drill bit. As a result, he suffered a partial amputation to the ring finger on his left hand with a small amount of bone loss.

An employee was cutting frozen pork with a bandsaw. The sawblade lacerated his right hand and partially amputated the thumb.

An employee was cleaning the fat puller end of a puller machine when she attempted to remove a piece of chicken fat from the plastic auger's point of operation. The plastic auger then amputated her right index fingertip.
