An employee was cleaning trash when he stepped on broken glass, leading to a laceration that became infected.

A letter carrier slipped on black ice while exiting a mail delivery vehicle. She fell and suffered a broken ankle.

An employee was using a scraper to clean a dryer chute. As the employee was cleaning the inside of the chute, the air lock blades amputated the employee's right middle, ring, and index fingers. The machine was running at the time.

An employee was driving a vehicle when another vehicle struck it. The employee suffered a compression fracture to a back vertebra, as well as a contusion to the right lung.

While walking at grade under an asphalt rail car loading rack, an employee tripped on concrete that was protruding from the walkway. The employee fell and sustained five broken ribs.

An employee was on a baker scaffold set at approximately 4 feet high, preparing the existing siding on a house to receive new siding at the eve of the porch roof addition. A temporary post that was supporting the roof's ridge beam was removed causing the roof structure to collapse, striking the employee. The employee sustained fractures to the left hip and left shoulder requiring surgery.

The injured employee was on the ground holding the bottom part of a sprayer while a coworker sprayed releasing agent onto concrete forms. The coworker noticed that the form was leaning toward the injured employee and shouted to warn him. The injured employee tried to run, but the form fell forward on his lower torso. The employee sustained a fractured hip.

An employee in the deli department was cutting tomatoes with a mandoline when the mandoline amputated their little fingertip.
