On September 29, 2025, a rural mail carrier was delivering parcels to a business when she tripped on a parking stop block in front of the business. She fell to the ground and sustained fractures to her left hip and pelvis. The employee was hospitalized and required surgery.

An employee was walking around chairs when they slipped/tripped and fell, resulting in hospitalization with a fractured hip.

An employee was walking on the sidewalk near the administrative building when he slipped on ice and fell to the concrete ground. The employee suffered a left hip fracture and was hospitalized.

A mail delivery employee was returning to his vehicle when he slipped on some patchy spots in a resident's driveway and fell to the ground, resulting in a fractured right leg.

An employee was pushing a cart loaded with 15 rounds of ammunition shells into an oven. The funnel broke off, causing an explosive to go off. The employee sustained burns on the right side of his head.

An employee was unloading freight from a truck at a dock. When she opened the trailer doors, a partial pallet of product fell on her, knocking her to the ground. The employee was hospitalized with a back injury.

On October 25, 2021, an employee was attacked by a dog while delivering mail, causing the employee to fall and strike their head on the ground. The employee suffered a fractured skull, bleeding on the brain, bite marks on the arm and leg, and a fractured right arm.

An employee was operating a ride-on pallet jack near a loading dock. The pallet jack drove over an uneven surface, causing the pallet jack to eject the employee onto the floor. The employee suffered a broken lower right leg.

An employee was on a workroom floor during a power outage, locking up a reasonable accommodation chair. The employee tripped on a guardrail that was lying on the floor, fell into another guardrail, and suffered a broken left shoulder and broken left-side ribs.

An employee was on a roof assisting with uploading materials and was also giving signals to a forklift operator. Insulation slid on the forklift's forks and struck the employee who then fell off the roof 12 feet to the ground. The employee suffered a back injury and a cut above his left eye.

An employee was working from a set of portable metal stairs, cleaning ammunition rounds with a drill brush. The wheel from a cart struck the stairs as the cart was pulled forward, causing the employee's leg to slip off the stairs and be caught between the stairs and a bollard. The employee suffered a broken tibia and fibula and was hospitalized.

On April 6, 2020, an employee climbed a 25-foot pine tree to complete routine maintenance trimming on a two-phase powerline. The employee completed the work on the southwest side of the tree, passed through the center, and began to reposition to the northeast side of the tree. While the employee was repositioning, the limb that he had his work positioning lanyard attached to failed and the employee fell approximately 20 feet to the ground, landing on his back near the base of the tree. The employee was hospitalized with a ruptured diaphragm, collapsed lung, multiple lumbar and thoracic fractures, laceration of the spleen, epidural hematoma, right renal contusion, and multiple rib fractures.

An employee had been dispatching APCs using a ride-on pallet jack and struck an I-beam. The employee was thrown from the pallet jack onto the floor and fractured their right ankle.

An employee was walking into work from her car on 02/03/2020 when she slipped and fell on ice in the parking lot, fracturing her leg.

On 1/22/20, an employee was walking up the icy sidewalk of a residential house when the homeowner's dog barked. The employee was startled and turned to see where the dog was, causing him to slip and fall on the sidewalk. He fractured his right ankle.

An employee was handling a rope (tagline) while removing a lid from a barge. A gust of wind moved the barge lid, causing the rope to jerk around the employee's thumb and resulting in an amputation of the right thumb.

After delivering mail to a residential house, an employee was walking down from the wooden steps when she fell to the ground, resulting in a fractured right ankle.

An employee was delivering mail to a residence when he was stung by a wasp or bee. The employee had a severe allergic reaction and went into anaphylactic shock which caused a heart attack.

An employee was driving a side-by-side ATV along a berm checking a levee for damage when he hit a rock and lost control of the ATV, causing it to roll. He was ejected from the ATV, breaking his left ankle and receiving lacerations to the head.

On May 3, 2019, an employee was placing pipe in a trench and was working in the vicinity of an excavator. A hydraulic line on the excavator ruptured causing the bucket to swing down and strike the employee. The employee sustained a fractured right arm and kidney damage.

A police officer was on a roof during his rounds. He slipped and a closing door ripped the fourth finger off his left hand.

While trying to remove a bolt from a concrete form, an employee suffered a pinched left ring finger. The fingertip was amputated, including bone.
