An employee fell from a dock to ground level (less than 4 feet) and broke their tibia/knee.

An employee was on the ground working to close a trailer door. The door was caught by the wind and blew the employee backward. He landed on his back and sustained four fractured ribs, requiring hospitalization.

An employee was walking on top of boiler to close a valve. The employee fell off the boiler and landed on the floor, sustaining fractured ribs. The employee was hospitalized.

An employee's left bicep was lacerated by high-pressure water during water blasting operations. The employee was hospitalized.

An employee was exiting a loader and coming down the ladder. His hand slipped off the railing and he fell backward onto sandy ground, landing on his side. The employee was hospitalized with a fractured pelvis and a rotator cuff tear.

An employee was working to remove a stuck fish tool from a drill shoe. Their thumb was struck by a hammer, resulting in amputation.

During a break, an employee was engaged in a conversation with a customer. They were standing approximately 7 feet in front of a combine header that was attached to a payloader but also set on jack stands for support. The header dislodged from the payloader, slid forward along with its support stand, and struck both the employee and the customer. The employee was hospitalized with fractures to thoracic and lumbar vertebrae, a fractured rib, and a partially deflated lung. The customer also sustained injury.

The cover for an HVAC chute had been removed so a metal platform could be placed on top. An employee fell through the opening to a surface about 20 feet below, landing on his back and suffering three fractured vertebrae.

An employee went to sit on a chair when it rolled out from under her, causing her to fall to the floor and strike her head. The employee was hospitalized with a closed fracture to a thoracic vertebra.

An employee was operating a steel rolling machine when their left fingertip became caught in the rollers, resulting in a fingertip amputation.

An employee had been removing grates and placing straps on a tank so the tank could be moved. The employee jumped off the tank and was caught on the interior brace inside the tank. His leg twisted and he fell to the ground, resulting in a fractured tibia and fibula.

An employee was restarting a compressor when he bumped into the side of a staircase and suffered a bruised leg. The employee was hospitalized.

Two employees were installing framing for a 10-foot-by-10-foot overhead door. They had pre-assembled the frame and were hoisting it up with a chain fall. The frame became bound up in the opening. As one of the employees was freeing it from a manlift, the frame was bumped and the choker that was rigged to the chain fall rolled out of the hook on the chain fall, bending the safety latch out. This caused the frame to tip over and strike the other employee. It knocked him down and pinned him to the floor from the left hip to the left shoulder, and he was hospitalized with fractures to ribs on the left side and his left hip.

An employee was removing contaminants from a low point drain on an ammonia line through a hose and into a container of water. A release of pressure from the end of the hose exposed the employee to anhydrous ammonia, resulting in chemical burns to his face, arm, and chest as well as ammonia inhalation. He was hospitalized.

An employee was operating a stand-up forklift when it struck a racking post and the employee's lower leg was crushed, resulting in an open fracture of the tibia and fibula.

An employee was loading a motor onto a trailer when he struck his left elbow against the radiator. His elbow was lacerated and became infected, resulting in hospitalization.

An employee was standing near a dump truck, watching as it dumped a load of rocks. When the end dump was put down, the employee noticed there was a rock stuck in its tailgate and lifted the tailgate so the rock would come out. The rock fell out and the tailgate caught his left ring finger. The finger was lacerated and broken. He was hospitalized, requiring surgery.

An employee was installing a metal rail when three fingers on their right hand were crushed between the rail and another object. The employee was hospitalized.

An employee hooked a casing tool cradle to a loader, which then picked up the casing tool. The casing tool swung around and knocked the employee to the ground. The loader then went to set the casing tool on the ground and it struck the employee's left leg, resulting in a crush injury that required hospitalization and surgery.

An employee was lowering a belt guard back onto a pumping unit when their left thumb was caught between the guard and the pumping motor. The employee sustained an amputation of the thumb at the first joint.

An employee was using a hand cart to move boxes upstairs when they fell, resulting in a laceration to the scalp and fractures to their neck and back.

An employee was assisting an operator with changing out an auger bit. The outer Kelly bar was lowered to put pressure down and free the pin. The employee's left little finger was crushed by the Kelly bar, resulting in amputation.

An employee on top of a moving railcar when their self-retracting lifeline (SRL) became caught on a load-out spout. The employee was dragged along the railcar, then rolled off the railcar and fell to ground. The employee was hospitalized with fractures to their wrist, right side ribs, and femur.

Employees were unloading a pallet jack from the forks of a forklift. The pallet jack dropped off the forks and an employee's right thumb got caught in the handle. The tip of his thumb was amputated.

An employee was removing a beet elevating belt while mechanics were cutting a hole in the center of the belt to install the pulley point using an oscillating tool with a hook blade. The tool popped out of the belt and the tool struck the injured employee in the stomach, resulting in a laceration on the lower left abdomen.
