An employee was working with an uncooperative patient when the patient fell onto their arm. The employee sustained two fractures to their arm and required surgery.

A nurse was working in the emergency department when they experienced nausea, difficulty breathing, and a cough due to chemical fumes. The employee was hospitalized.

An employee was using a table saw to make wooden door stoppers when the wood kicked back and the blade caught part of their left hand, impacting two fingers and amputating one finger. The saw was unguarded at the time of the incident.

At about 1:20 p.m. on September 26, 2022, an employee was inspecting a house that was being remodeled when he fell through an opening on the second floor where stairs were going to be constructed. He struck some scaffolding and landed on the first floor. He suffered injuries to the right wrist, right elbow, left leg/knee, neck, and mid- to lower back. He also suffered five broken left-side ribs and bruised/collapsed lungs.

An employee driving a yard truck was parking the container when the container began to overturn, causing the cabin to bounce several times with the employee inside. He was hospitalized with three broken ribs, internal bleeding in his liver and head, and bruises on his face and head. He was hospitalized.

An employee was hospitalized when he suffered electrical shock while servicing a school's HVAC system, causing burns to his hands and chest pains.

An employee was operating a yard truck. The truck cabin was lifted by a crane and the employee fell out, landing on the hard concrete below and suffering a broken collarbone and multiple hematomas.

An employee was walking outside the premises of the main building. The soil foundation was loose and he stepped over vapor lines under the ground, resulting in his left leg getting burned from the calf down. He was hospitalized.

On April 30, 2020, an employee was removing torque rods from a truck tractor using a 25-pound sledge hammer when the sledge hammer struck his left third finger, lacerating it and requiring surgery and hospitalization.

While signaling a crane operator, an employee fell through an opening and landed on the floor of a container ship, about 25 feet below. He suffered fractures in his leg, hip, ribs, and right arm.

An employee was walking behind a top loader when the top loader operator reversed the equipment to pick up a container. The employee's right foot was struck and caught on the top loader wheel, resulting in an amputation below the knee.

An employee was driving a truck transporting a 40-foot container. The employee was making a turn when the container fell sideways and the truck overturned. The employee sustained a broken right femur inside the cab.

On 05/17/2018, an employee finished unloading a boat in the yard area and was walking off a travel lift when he tripped on a piece a wood, fell, and hit his head on a concrete pedestal. He sustained head and neck trauma, requiring hospitalization and stitches.

An employee was doing maintenance on a piece of equipment that was shut down. He grabbed the motor's damper/pulley belt and pulled the belt toward himself. This caused the damper/pulley to rotate, pinning his right-hand thumb between the damper/pulley and the belt. As a result, the top part of his right-hand thumb (approximately 1/8" to 3/16") was amputated.

An employee fell 12 feet while climbing a ladder to gain access to a scaffold work platform. The employee was hospitalized due to multiple fractures.

On 03/24/15 at 3:05 a.m., an employee was driving a yard truck with a container attached. While maneuvering, the container flipped over and disconnected from the yard truck. The truck bounced, and the employee suffered head trauma, tooth loss, and an ankle fracture.
