The employee attempted to attach the other end of this chain to a steel loop (hasp) on the back (underside) of an excavator bucket when the excavator boom unexpectedly dropped, pinning the employee's right thumb between the bottom of the excavator bucket and the concrete lid. His right thumb was amputated at a point between the first and second knuckle.
