A forklift was used to load a flammable storage cabinet (weighing approximately 265 pounds) into the bed of a pickup truck. Once the forklift was removed and parked, employees went to re-position the cabinet. They lifted one edge of the cabinet to roll it on its side. The cabinet dropped approximately 2 inches and landed on the injured employee s left middle finger causing a fingertip amputation.

While climbing down a military vehicle (MMPV) access ladder, the employee's wedding ring on his left hand got caught on one of the anti-slip barbs on the step. When he dismounted the last step, the flesh was pulled from his ring finger, resulting in its ultimate amputation.
