An employee was changing out the tong heads in the power tongs on a drilling rig when they became energized and crushed the employee's hand.

An employee was securing an injector head to a cradle for transport when the catwalk above him slammed down. The catwalk crushed/severed his left middle finger which was caught in the pinch points where the catwalk came to rest, resulting in a partial amputation.

An employee was standing on an injector head feeding chain with a rope, when the rope broke, causing him to lose his balance and fall to the ground. The employee sustained fractures to his left elbow and three ribs.

An employee was changing rams in a blowout preventer on a snubbing unit. A nearby co-worker handed the employee a block when it slipped and fell onto the employee's left little fingertip, resulting in amputation.

An employee and a co-worker picked up a 48-inch steel pipe wrench to size up a saver sub. While sizing up the wrench, the swivel engaged and the pipe wrench hit the saver sub, causing the wrench to swing around and strike the employee in the left arm and chest area. The employee suffered a lacerated spleen.

An employee was performing an inspection of the seed warehouse when they fell 30 feet from a catwalk to the ground. The employee sustained facial injuries and a compound fracture to the left femur.

An employee was on a cherry picker approximately 30 feet high, selecting boxes of merchandise from warehouse racking to place onto a pallet. The employee fell and sustained neck and back injuries.

On November 6, 2023, an employee was installing roofing materials on a multi-residential construction site 35 feet above ground level. The anchor on the employee's personal fall arrest system became unattached and the employee fell to the ground. They sustained multiple fractures.

On October 12, 2023, an employee was stacking insulation on a roof when they fell 35 feet through a skylight to the concrete floor below. The employee sustained an ankle and pelvis fracture.

On the fifth or sixth floor deck, an employee was supervising iron workers who were measuring to place rebar. The employee was struck by a wooden tool box that was being lifted by a tower crane. The employee then struck a wooden fence and fell approximately 50 feet to the ground. The employee sustained fractures to both legs and lacerations.

An employee was slipping pipe when it became stuck and the pipe snapped, striking the employee in the back and ribs. The employee sustained fractured ribs.

An employee was checking voltage and fuses to see where power loss was occurring. He touched a magnetic coil and was shocked.

An employee was conducting maintenance on a pipe and was using a T-bar tool to secure the retainer cap on a pump. The employee's left index finger was caught in the rotating pump, resulting in a partial amputation.

An employee was closing a door when the door closed on their right index finger, resulting in a fingertip amputation.

An employee was walking into the motor control center (MCC) room when his right ring finger was caught in the hinge of a doorway. He sustained an open phalanx fracture, which resulted in a partial amputation above the first knuckle.

An employee was changing the spacing on a telehandler's forks. A fork slipped, and the employee's left index finger was caught between it and the mast. The fingertip was medically amputated at the first knuckle.

An employee was pulling down a broken skid with a forklift. When the employee backed up the forklift to get the forks out of the skid he pulled down, he contacted the forks of another parked forklift, fracturing both of his legs. He was hospitalized.

An employee was carrying cups back to the kitchen when her foot got caught on a cart and she fell face-first. During the fall, a piece of glass from a cup cut the inside of her mouth, severing an artery. She also sustained a laceration on her lower lip. The employee was hospitalized.
