The employee was climbing up a railcar ladder when he lost his balance and slipped 4 to 5 feet off of the railcar breaking a wrist, injuring his pelvis, and lacerating his forehead.

The dock plate slipped and the employee went to adjust the plate with their hand when their left middle finger got caught. Their finger was amputated above the first joint.

An employee was rotating a carbide granulator blade when his finger was caught between the blade and a stationary part of the machine. The employee's right middle finger was partially amputated and they suffered an avulsion to the right ring fingertip.

As the employee was manually spinning the spindle, his index fingertip was crushed between the spindle's jaws and the grooving insert, resulting in a fingertip amputation.

On October 30, 2025, an employee was working to adjust a stackable metal shipping container. As the container dropped into place, it caught the employee's hands in an area between the upper and lower cross-members. The employee suffered a laceration to the left ring finger that required stitches, bruising to the right ring finger, and fractures to the right middle finger that necessitated medical amputation of the fingertip.

A temporary employee was punched in the face by another temporary employee. He suffered a laceration to the eye.

An employee was backing up a powered industrial vehicle. Her left hand was caught between the vehicle and a rack, and the wrist was broken.

After rewiring the new spool, the employee's clothing got caught in the turning shaft when the left spindle started back up. The employee's left arm was pulled into the machine, resulting in a fractured humerus and lacerations to her triceps. She was hospitalized and required surgery.

An employee was resetting a warehouse racking system utilizing an order picker. They fell approximately 5 feet from the order picker to the concrete floor and sustained fractured ribs.
