An employee was preparing to load two scissor lifts onto a truck. He had loaded the first one, and was driving the second one up the ramp of the truck, when he and the lift fell off the truck to the ground about 6 feet below. He suffered a broken femur and a broken arm.

An employee was servicing a forklift at a jobsite and changing out the forks when he lost his balance after picking up the first fork. He stumbled and fell backwards to the ground, fracturing his right arm.

An employee was stepping out of a car when they slipped and fell on ice, resulting in a femur fracture.

An employee was closing up for the weekend when they slipped on oil and brake fluid and fell. The employee suffered a leg fracture.

An employee was working in the forest flagging an area to be harvested. He took a step and his foot slipped on a stick hidden under the leaves, causing him to fall to the ground. He landed on his right foot/lower leg resulting in a fractured tibia.

An employee was inspecting an apartment to ensure it was ready for a new resident to move in. The carpet in the apartment had been recently cleaned and was still wet. As the employee went from the carpet to the tiled floor of the bathroom, they slipped and fell, resulting in a torn left hamstring.

An employee was delivering a letter along a rural carrier route when she stepped on an ice-covered snow drift, slipped, and fell to the ground. The employee sustained a right hip fracture that required surgery.

An excavator bucket was being removed. An employee was pulling a snap ring to remove a pin and lost his grip. The high-tension snap ring recoiled and his left middle fingertip was amputated by the snap ring.

While using the remote operator controls and walking alongside the lift, the wheel of the lift turned in the wrong direction, causing it to strike the employee and roll over his left ankle. The employee was hospitalized with two broken bones in his ankle.

An employee was working on an excavator's tracks. He was swinging a sledgehammer to finish pushing a metal tractor pin into the track links. A hammer-blow caused a piece of the pin to fly off, ricochet, and embed itself in his buttock. He was hospitalized, requiring surgery.

An employee was picking up crane pads to put onto their trailer. Their finger got pinched between pads, resulting in an amputation involving the loss of skin.

An employee was delivering materials. He was doing his pre-delivery inspection on the roof of the jobsite (a warehouse) when he fell through a plexiglass skylight. He landed on his feet on the concrete floor 14 feet below. The employee sustained fractures to his spine, left ulnar/radius, and right heel. The employee required surgery.

An employee was standing on an extension ladder, using a torquing tool to remove bolts that secured blades to a rotor. When the torquing tool activated, its reaction arm came around and pinched the employee's right middle finger against a lifting eye. His fingertip was amputated.
