On January 23, 2015, at approximately 6:30 p.m., a worker from the inside machine shop was training another employee on how to sand the face of a flange using a sanding disk mounted onto the tool rest of a lathe. The flange (part of a very long shaft) was on a lathe rotating at approximately 20 rpms. When the worker went to adjust the sanding disk, his left pinky at the first knuckle joint was amputated by one of the holes in the flange.

An employee was aligning a piece of 1 1/2-inch schedule 80 pipe into a rotary draw bender when the chuck was moved beyond the table. This caused the chuck and pipe to fall from the table. The employee's right index and middle fingers were pinched between the pipe and the table, resulting in amputations.

An employee was transiting down the ships stairs leading into a winch room aboard a dredging ship. The employee fell down the stairs and impacted a vertical pipe that was installed adjacent to the bottom of the stairs. The left side of the employee s midsection was impacted causing internal injury to the stomach and diaphragm.

An employee was hoisting a 5,000-pound steady rest with a 50-ton overhead shop crane. The load fell on his left leg, crushing it from the knee down. The leg was medically amputated.

An employee was in a lift with a blasting hose that was tied to the lift basket so the employee could work at an elevated position. During this time, the hose was charged, activating the discharge of shot blast. This caused the hose to whip around violently while discharging grit. The blasting nozzle struck the employee's face and shot blast was embedded in his face.

An employee slipped on concrete in a wash basin and attempted to prevent his fall by grabbing onto a section of portable scaffolding. The scaffolding fell on top of him and he struck the concrete, suffering a head injury, concussion, and brain bleed.

The employee was hospitalized.
