An employee slipped on concrete in a wash basin and attempted to prevent his fall by grabbing onto a section of portable scaffolding. The scaffolding fell on top of him and he struck the concrete, suffering a head injury, concussion, and brain bleed.
An employee was aligning a piece of 1 1/2-inch schedule 80 pipe into a rotary draw bender when the chuck was moved beyond the table. This caused the chuck and pipe to fall from the table.
