On 2/25/2015, an employee's left index finger was caught in a conveyor belt that was being used to move shingles to a rooftop and crushed but not severed.
After loading shingles, an employee was unhooking from the roof to come down. He fell to the ground while wearing fall protection and sustained fractures to his lower right arm and left heel.
