After loading shingles, an employee was unhooking from the roof to come down. He fell to the ground while wearing fall protection and sustained fractures to his lower right arm and left heel.

An employee was climbing a ladder from a driveway to a roof. The ladder kicked out from under him and he fell roughly 8 feet, landing on the ladder where it lay in the concrete driveway. He suffered fractures to the right forearm and wrist.

The employee was loading a skid onto the trailer and as they were lowering it there was a piece of wood stuck on the underside of the skid.
