An employee was walking along a catwalk while tracking a section of chain when they fell 7.5 feet to the ground below, resulting in a broken rib, punctured lung, broken collarbone, broken wrist, laceration to head, and a brain bleed.

An employee and a co-worker were removing forks from an unused forklift in order to remove the fork positioners. They removed a pin from the fork and it shifted and swung toward the employee. The employee attempted to jump over the fork, but it struck his foot mid-air, resulting in fractured bones on the top of his foot.

An employee went onto a rollcase to try and release a board that was trapped inside the outfeed rollcase of an edger machine. He lost his balance and fell 10 feet onto a steel waste chute, then slid an additional 10 feet to the bottom of the chute. He suffered a fractured pelvis and broken ribs, as well as a laceration above the eye.

On June 10, 2024, an employee was grinding mulch in a tub grinder. A front end loader was unloading mulch into the tub grinder and ran over his ankle, breaking it and also causing a hairline fracture to his pelvis. He was hospitalized, requiring surgery.

On January 17, 2024, an employee was working on a roller machine when a piece of metal lacerated their right forearm, causing tendon injury. The employee was hospitalized and underwent surgery.

An employee had been warming his hands at a stationary heater near the stacker operator. He was by the rear passenger-side of his parked forklift when another forklift reversed and struck him. His left leg was pinned between the two forklifts and he sustained a fractured fibula.

An employee was operating an ironworker (steel punching/shearing) machine. A piece of the tooling broke off and struck the employee's neck; a fragment was lodged in his neck. The employee was hospitalized and required surgery.

On October 25, 2025, an employee was stringing up a new reel of tire tread on a machine. The employee pulled a 6-inch tail and proceeded to spool up the liner. As the machine was jogged to begin the liner wrapping process, the liner caught the employee's glove and pulled their right arm into the liner, causing a fracture to the arm.

Big River Steel, LLC

An employee was doing a cable change on top of an electric overhead traveling (EOT) crane. They were positioned between the cable drum and the drive shaft. While rotating the cable drum, the employee reached for the new cable that was to be installed. The grease fitting on the drive shaft caught their fall harness and pulled them underneath the drive shaft. The employee was pinned between the drive shaft and two pieces of angle steel, resulting in a fracture to their left hip.

An employee was helping two other employees to push a pallet of tile on a loading dock. He suffered a broken fibula and was hospitalized.

On October 5, 2025, an employee was pulling a pressure washer hose when it it disconnected from the cooker. The employee was struck by jets of high-pressure water, resulting in lacerations to their left thigh requiring surgery. The employee was hospitalized.
