An employee was cleaning out a scrap box. The employee waved to the PIT operator to reverse. The operator started to reverse when his foot slipped. The PIT moved forward and pinned the employee between the scrap box and a guardrail.

On December 30, 2023, an employee was pulling a pallet into the back room when a forklift rounded the corner and ran over the employee's right foot. Three of the employee's toes were amputated.
