Employee was walking in the very center of 10-feet tall racking on plywood and then stepped down to the next level on a piece of plywood that had no center support. It collapsed and he fell 6 feet to the next level.
He was disassembling the racking system just before he fell
Two employees were loading large rolls of material onto a trailer. One employee was using a forklift. He unloaded one roll of material and it rolled across the bed of the trailer and fell off the other side, bouncing into the other employee's leg and fracturing his right fibula.
