THE PEOPLES GAS LIGHT AND COKE COMPANY

On November 2, 2023, at approximately 1:00 p.m., an employee was lighting the pilot on a customer's furnace when they sustained first- and second-degree burns.

An employee was walking at a jobsite and stepped on a yellow composite sidewalk cover. The cover became dislodged and he fell into an excavation that was 46 inches deep. The employee sustained injury to their left knee and lower back.

On August 19, 2021, an employee was unloading a pipe storage rack from a flatbed trailer. A chain was holding two sections of pipe rack together. When the chain was removed, a section of pipe rack became unstable, causing the rack and the employee to fall approximately 3 feet from the flatbed trailer to the ground. The employee became pinned under the section of pipe rack and was hospitalized with a fractured pelvis. The section of pipe rack was 30 feet long and weighed approximately 1,380 pounds.

An employee was stepping out of a car when they slipped and fell on ice, resulting in a femur fracture.

An employee was closing up for the weekend when they slipped on oil and brake fluid and fell. The employee suffered a leg fracture.

An employee was working in the forest flagging an area to be harvested. He took a step and his foot slipped on a stick hidden under the leaves, causing him to fall to the ground. He landed on his right foot/lower leg resulting in a fractured tibia.

An employee was inspecting an apartment to ensure it was ready for a new resident to move in. The carpet in the apartment had been recently cleaned and was still wet. As the employee went from the carpet to the tiled floor of the bathroom, they slipped and fell, resulting in a torn left hamstring.

An employee was delivering a letter along a rural carrier route when she stepped on an ice-covered snow drift, slipped, and fell to the ground. The employee sustained a right hip fracture that required surgery.

An employee was operating an aerial work platform. While the employee was maneuvering the lift through a set of double doors, their head was caught between the door frame and the platform's vertical mast. The employee suffered a skull fracture that required surgery.

An employee was transporting used filter media (containing pyrophoric iron sulfide) from an inlet gas separator in the bed of a truck. The material ignited. While the employee was removing items from the truck bed, a gas can was exposed to the fire and off-gassing vapors ignited. The employee suffered first- and second-degree burns to both arms, the chest, the neck, and the right ear. The employee was hospitalized.

An employee was using a pipe threading machine to thread a 2-inch diameter gas pipe during the rebuilding of a gas meter. The pipe was in a vise mounted on the back of the crew truck. A few pieces of metal chips or shavings caused the threading machine to jam. The employee started to back the pipe up, but it jammed in that direction as well. While reaching to pull the handle back up into place to try and start again, the machine rotated about an inch and pinched his middle fingertip against the edge of the truck s back step. The employee's fingertip was partially amputated without loss of bone.

The injured employee was manually lowering a ramp on a heavy equipment trailer with the assistance of another employee. The injured employee was standing behind the trailer ramp. After removing the safety pins, the ramp descended quickly and struck the injured employee, causing them to fall to the ground. The employee was hospitalized with fractures to their sternum and L1 vertebra.

An employee was lighting a tankless water heater when an explosion occurred and the employee was burned.

An employee fell while moving a water canister, resulting in fractures to four fingers on her right hand.

An employee was changing a die in a press when the die slipped and crushed the employee's left index finger. The employee sustained an open facture of the tuft of the left distal phalanx and a partial amputation.

An employee was mixing sauce using a sauce barrel mixer when his left little finger was amputated.

An employee was driving a boom lift (in the lowered position) in an exterior dock area. The lift s left wheels rolled onto base plates that covered a 3-foot-deep sump pit. The base plates failed, and one side of the lift dropped. The employee's left leg was caught under the lift basket, and he suffered a fracture to the lower leg including the ankle.

A temporary employee was carrying a nail gun through the warehouse when it hit his leg, activated, and fired a nail into the leg.
