An employee was using a pipe wrench to loosen a piece of pipe above a 2-inch swedge. The wrench slipped and he lost his balance and fell backward, turning as he fell. His left side came into contact with a guardrail behind him and he suffered four rib fractures. He was hospitalized.

Employees were preparing a pig receiver and pig launcher. The block valves had been closed, and the receiver and launcher had been blown down; liquids were draining from the opening of the launcher. A forklift was being used to open the receiver hatch. The hatch struck the forklift's forks, and liquids at the receiver and launcher ignited. An employee suffered second-degree burns to the face and right hand, as well as first-degree burns to the left and right lower legs, and was hospitalized.

Two employees were working to clear land with other employees and prepping a brush pile (20 x 40 x 15 feet) to be burned. They planned to ignite the brush pile by creating a fire-line or trail with an accelerant so that the pile could be lit from a distance. After applying a gasoline-diesel mixture to the pile, one employee used a cigarette lighter to ignite the pile. Two employees suffered multiple first-degree burns to the face, ears, hands, chest, and abdomen.

An employee was hot patching a tire, which involves lighting a flammable liquid on fire to patch the tire. The flammable liquid contacted his arm, and he sustained burns to his arms and face.

An employee was using a cutting torch to heat up bolts that had seized up. The bolts caught on fire in a flash burn, and the employee suffered burns to the arms and elbows.

On August 2, 2025, at 4:59 PM, a contract painter was painting an aircraft using an electrostatic paint gun in the paint booth hangar. He was placing the gun into a container of methyl ethyl ketone to clean when the solvent ignited. The employee was hospitalized with third-degree burns to his arms and legs.

An employee was outside, participating in fire extinguisher training onsite. She extinguished a fire and then the chemical from the fire got in her face. She started coughing and was hospitalized with trouble breathing.

An employee was operating an aerial work platform. While the employee was maneuvering the lift through a set of double doors, their head was caught between the door frame and the platform's vertical mast. The employee suffered a skull fracture that required surgery.

An employee was using a pipe threading machine to thread a 2-inch diameter gas pipe during the rebuilding of a gas meter. The pipe was in a vise mounted on the back of the crew truck. A few pieces of metal chips or shavings caused the threading machine to jam. The employee started to back the pipe up, but it jammed in that direction as well. While reaching to pull the handle back up into place to try and start again, the machine rotated about an inch and pinched his middle fingertip against the edge of the truck s back step. The employee's fingertip was partially amputated without loss of bone.

The injured employee was manually lowering a ramp on a heavy equipment trailer with the assistance of another employee. The injured employee was standing behind the trailer ramp. After removing the safety pins, the ramp descended quickly and struck the injured employee, causing them to fall to the ground. The employee was hospitalized with fractures to their sternum and L1 vertebra.

An employee was lighting a tankless water heater when an explosion occurred and the employee was burned.

An employee was performing a pipe count while a pipeline contractor was unloading steel pipe at a staging yard. The contractor was preparing to set a pipe segment (12 diameter) on the top row of a pyramid-shaped stack using an excavator. The contractor was making his final adjustments before setting the pipe when the injured employee (Enbridge Inspector) went to get label information from one of the pipes in the stack. The pipe stack shifted as the contractor began to position the pipe, and the bottom piece (42 feet long and 2,083 pounds) rolled onto the injured employee s lower left leg, trapping the employee underneath it. The employee was hospitalized with a compound fracture of his tibia and fibula and he required surgery.

An employee was working to deliver mail to an apartment building. She was waiting for a customer to move, to obtain clearance to the mailboxes. The door swung inward and closed on her right little finger. The top half of her finger was surgically amputated.

An employee fell from a step ladder while reaching for a tie-off point. He impacted the ground, and landed on his bolt bag which contained tools. The employee was hospitalized with fractured ribs, and a lacerated spleen and kidney.

On October 30, 2025, an employee was working to adjust a stackable metal shipping container. As the container dropped into place, it caught the employee's hands in an area between the upper and lower cross-members. The employee suffered a laceration to the left ring finger that required stitches, bruising to the right ring finger, and fractures to the right middle finger that necessitated medical amputation of the fingertip.

An employee was moving a scissor lift through a doorway. The employee was pinned between the scissor lift and the doorframe, sustained a back injury, and was hospitalized.

An employee was setting up communication equipment for a meeting. They were walking and tripped over a speaker on the ground. The employee sustained a leg injury.
