An employee was adjusting a paint roller when his left ring finger was caught in a chain and sprocket. The fingertip was amputated.

An employee was operating a slab line in a plant that produces 16" x 9" x 3" concrete slabs that weigh approximately 25-30 pounds. The employee went to adjust the chain and sprocket for the doubler that stacks the slabs two high, when their right middle finger was caught in the machine. The employee sustained a partial transphalangeal amputation of the finger.

Two employees were shoveling out a machine. The blade of a shovel struck the injured employee's left middle finger, causing a partial amputation.

An employee was behind a brick firing machine aligning bricks for production when the employee was caught between the pusher bar and the setting head, resulting in a fractured pelvis. The machine was energized and was not locked out/tagged out at the time.

On April 15, 2024, an employee was trying to adjust a brick on a machine that was pushing brick out of a building. The machine engaged and pinned his right leg against a cube of brick. His leg was broken and he was hospitalized, undergoing surgery.

An employee was changing mud cups in the hopper cylinders of a concrete pump truck. The cylinders moved and three of the employees fingers were amputated.

An employee was standing on the rig floor next to a polishing unit. His hand was placed on the polishing unit when the pump was lowered, resulting in amputation of their right thumb, ring, and middle fingertips.

An employee was using air to blow out the air wash to the blender and grinder station. As the employee reached to verify that the air wash was clean, the unit pinched and amputated their fingertip.

An employee was retrieving a lost drill pipe with a lifting bail when their left thumb got pinched between the table and handle of the lifting bail while trying to re-thread the pipe to lift it out. The employee suffered an amputation to the left thumb.

An employee was removing a core chuck from a stationary roll when their right ring finger was pinched between the chuck and the roll. The employee's fingertip was amputated.

On September 30, 2025, an employee was applying production labels to a splintered piece of pallet on a conveyor. Their right middle finger was caught as the split line exit conveyor moved forward, resulting in an amputation.

An employee was operating a pattern block roller machine. The employee was pinned (at the mid- to upper torso) between the pusher bar and the assembly platform. The employee suffered broken ribs.

Employees were chipping concrete/brick to separate a building for demolition when the wall collapsed and debris fell onto the employees, resulting in hospitalization of two employees. One employee sustained a fractured left femur and a head injury, and the other sustained a severe head injury.

An employee was removing packaging from a roll of printing substrate. The blade of their utility knife got stuck. The employee used both hands to free the knife and the blade partially amputated their left little fingertip.

A concrete batch plant operator was assisting with clearing spoil piles using a skid steer. The skid steer backed into a stationary screen plant. The employee's left little finger was crushed between the controls of the skid steer and the screen plant, resulting in a fracture and laceration. The employee's finger was surgically amputated.

An employee was standing on an extension ladder, using a torquing tool to remove bolts that secured blades to a rotor. When the torquing tool activated, its reaction arm came around and pinched the employee's right middle finger against a lifting eye. His fingertip was amputated.
