The driver drove a chassis with a container up to a crane to have the crane lift the container from the chassis. The pins were not disengaged from the chassis and the crane picked up the chassis and the yard jockey (half-cab). An employee was lifted and dropped and sustained two broken vertebrae.

An employee was trying to connect a cargo box to a railcar when a crane operator moved the cargo box and it pinched the employee's right index finger, resulting in an amputation.
