On 02/26/2015, at approximately 10:30 AM, an employee picked up an electric handsaw that was energized. The employee activated the handsaw with his right hand and amputated the middle finger from his left hand.

Mako Contracting, LLC

An employee was using a saw to cut a piece of concrete when the piece gave way and pushed the saw into the employee's leg. The employee's leg was lacerated, requiring stitches and hospitalization.

East Penn Manufacturing

An employee was using a utility knife to cut honeycomb paper when the knife amputated their left index fingertip.

Ash Brothers Fireplace and Chimney Co

An employee was cleaning a resident's chimney. While using a drill, the employee's glove became caught in the drill and their finger was fractured.

An employee was grinding some hardfacing on a bulldozer blade when the 9-inch grinder kicked back and lacerated his left leg above the knee. The employee was hospitalized and required surgery.

An employee was operating a rotating tool to place thread protectors on the end of pipe. Her left hand was on the rotating part of the tool when the tool engaged, resulting in a left forearm fracture and a thumb laceration.

An employee was fixing a broken purlin at a worksite when they fell from the framing to the concrete floor approximately 19 feet below. The employee sustained brain bleeding and fractures to their face, vertebrae, and wrists.

Pen Gulf, Inc.

Employees were removing scaffolding that was used to repair an ion exchange tank. The line was undergoing the regen process when a piece of scaffold struck a PVC pipe that transported hydrochloric acid (HCl). The injured employee was placing material inside a scaffold rack when he was sprayed in the face with HCl and sustained chemical burns to his eyes, face, chest, right arm, and left leg.

Samsung E&C America, Inc.

An employee was moving a ramming machine to a staging area. While using the controls on the side of the machine, there was a sudden heavy rainfall causing the equipment to slide. The ramming machine tipped over and caught the employee against the ground. The employee sustained a fracture to the right side of their pelvis and was hospitalized.

United Dairy Farmers

The injured employee was training a new employee on a dough machine. She went to show where the batter comes from and her right index fingertip was cut. The employee sustained an amputation to the fingertip. The machine was not guarded at the time of the incident.

MVA CONSTRUCTION LLC

An employee was operating a roller machine on a dirt slope when the machine hit a bump and fell onto it's side. While trying to get free of the falling machine, the employee's legs were pinned by the top bar of the cage. The employee was hospitalized and his left leg was amputated. He also sustained fractures to a hand and finger.

An employee was setting up for a door installation on a three-story building. The employee was moving an aluminum track frame from one condo to the condo next door by using the balcony when they misstepped and fell between balconies. The employee landed on the dirt below and suffered facial fractures.

Caribbean Auto Mart

An employee was replacing a fuel pump on a pickup truck using a transmission jack to lower the fuel tank from the vehicle. When he removed and lowered the tank, the tank caught fire, resulting in burns to his hands, arms, and legs.

GEC, LLC

An employee was removing concrete forms from a current pour when a form fell from the wall. An intact pin hooked the employee's safety vest and pulled them from the scaffold to the ground, resulting in a bruised chest (lungs).

High Quality Concrete

An employee was driving a cement mixer. It flipped while turning a corner, and the cabin was flattened. The employee was pinned in the cabin and suffered a broken right hand, three broken ribs, and head lacerations.

Vivot Equipment Corporation

An employee had attached a crane sling to an 8-inch pipe, and the crane was holding the pipe with minimal tension while bolts were removed from its lower flange. The pipe came loose and knocked the employee off the trailer to the ground 6 feet below. The back of the employee's head struck the trailer after the fall, causing a head injury.
