An employee was pushing a cart containing metal materials that were approximately 25 feet in length. A wheel on the cart detached causing the cart to tip over. The employee's right little finger was pinched by the cart, resulting in partial amputation of the fingertip.

An employee was positioning an alloy bar in a vertical band saw. His finger became caught between a horizontal clamp and the alloy bar and sustained an amputation.
