Final narrative
An employee amputated part of his left thumb while using a masonry table saw to cut 4'x8' brick. The saw was guarded at the time of the incident.
On January 11, 2020, an employee was on the ground guiding a precast panel that was being set by a forklift/tractor. He slipped into a small hole and the forklift/tractor ran onto him, breaking his left foot and ankle. He was hospitalized.
An employee was using a Skilsaw to cut wood to form a sidewalk. The wood got caught in the saw and pulled the employee's hand into the saw. The employee's left thumb was cut off at the first knuckle.
An employee poured water into the concrete mixer and the water splashed into his eyes. Moments later, the employee extended his hand to pick up the bucket when he instead felt a tick on the tip of his left middle finger. His finger got nipped by the concrete mixer paddles. No amputation occurred.
An employee was operating a strip saw. As the employee shut off the saw, their right index and middle fingertips were amputated by the blade before it came to a complete stop.
