An employee was cleaning glass from under a conveyor belt using a broom. Her left arm was caught and pulled into the conveyor roller, where it was trapped. The arm was broken, as was a finger on her left hand.

An employee was using a pedestal grinder to grind a piece of square metal stock that was 1/4 inch in diameter by 13 inches in length.
