An employee slipped on a truck's lift gate and fell, suffering broken ribs and a ruptured spleen. The employee was hospitalized.

DAWN FOOD PRODUCTS INC

On 2/25/15 at approx. 5:15 p.m., an employee was sanitizing the muffin batter depositor pump. She removed the cover off the side of the equipment, sprayed inside, and then reached into the pump with her right hand to check for remaining batter mix. As a result, three fingertips on her right hand were amputated. The machine was not locked out at the time.

Employees were assembling and testing a butterfly valve unit and actuator combo. When the unit was function tested with air, the valve gate closed on an employee's left thumb resulting in a fracture and laceration.

An employee was doing a daily inspection of the sifter screen on a mixer when the mixer line activated. The armature for the screen struck the employee's right wrist. Their wrist was cut and their hand was fractured.

An employee was being trained on a machine when a roller came down on their hand, resulting in a fracture and laceration.

Flowers Baking Company of San Antonio LLC

A team was disassembling a dough pump s auger to allow the wastewater to drain from an allergen changeover. The auger became stuck due to suction caused by residual wet dough. A paddle was being used to pry the auger loose when the suction force suddenly released, causing the auger to retract unexpectedly. A finger on the employee's left hand was caught between the auger and the housing, leading to an amputation to that finger.

On February 4, 2025, at approximately 6:20 PM, an employee was cleaning the dough inside a divider when they sustained an amputation to their right index fingertip.

An employee was troubleshooting a mixer on the biscuit line when their hand became caught under the machine belt, resulting in fractures and an amputation of their middle fingertip. The machine was not locked out/tagged out at the time.

An employee was delivering materials. He was doing his pre-delivery inspection on the roof of the jobsite (a warehouse) when he fell through a plexiglass skylight. He landed on his feet on the concrete floor 14 feet below. The employee sustained fractures to his spine, left ulnar/radius, and right heel. The employee required surgery.

An employee was cutting cabinet trim using a table saw when four fingers on his left hand were severed, resulting in hospitalization and amputation.

An employee was removing packaging from a roll of printing substrate. The blade of their utility knife got stuck. The employee used both hands to free the knife and the blade partially amputated their left little fingertip.

A concrete batch plant operator was assisting with clearing spoil piles using a skid steer. The skid steer backed into a stationary screen plant. The employee's left little finger was crushed between the controls of the skid steer and the screen plant, resulting in a fracture and laceration. The employee's finger was surgically amputated.

An employee was standing on an extension ladder, using a torquing tool to remove bolts that secured blades to a rotor. When the torquing tool activated, its reaction arm came around and pinched the employee's right middle finger against a lifting eye. His fingertip was amputated.
