The injured employee was about to start loading a truck from the mattress staging area which was approximately 7'. The mattress was standing upright and lined up to form a wall to be loaded onto the truck. As the injured employee step out of the staging area (blind spot), another associate operating the forklift, could not stop in time and ran over the injured employee's left foot.

An employee used an attached rope to pull a door down and close it, then placed his foot on the foot bar to lock it. The rope looped around his foot. When he began to walk away, he tripped and was swung around, causing his left foot to come out from beneath him. He fell onto his right hip and broke it. He was hospitalized, requiring surgery.
