An employee was driving a stand-up counterbalance forklift down an aisle in a warehouse. The forklift abruptly turned; the employee's leg moved and the dead man switch activated, throwing the employee out of the forklift. Their right leg was crushed between the material-handling equipment and a racking system, and it was amputated above the knee.

An employee used an attached rope to pull a door down and close it, then placed his foot on the foot bar to lock it. The rope looped around his foot. When he began to walk away, he tripped and was swung around, causing his left foot to come out from beneath him. He fell onto his right hip and broke it. He was hospitalized, requiring surgery.

An employee was exiting a trailer after putting a label on a skid inside the trailer. The trailer began pulling away and the employee fell to the ground. The employee sustained fractures to their ribs on the right side, right kneecap, and a left toe.

The injured employee was sitting in a parked forklift while another forklift was moving two metal tire racks. The top rack was empty and the bottom rack contained tires. As the racks were being lowered to the ground, they fell and the top rack struck the injured employee. The employee was hospitalized with lacerations and a fracture to the left foot, and they required surgery.

An employee was attending a training course at the corporate office. At the end of the course, the group went outside to a patio to take a photo. The employee slipped off the patio stairs and fell to the grass-covered ground at the same level. He was hospitalized with a broken femur and broken hip.

An employee was driving a counterbalance forklift down an aisle when the forklift began veering toward a rack. Their right little finger was pinched between the side pillar of the forklift and the rack, resulting in a partial amputation to the finger.

An employee was riding on the side of an all-terrain forklift when they fell into the space between the gas tank and the right rear tire. The employee was run over by the the right rear tire, resulting in fractures to their neck, pelvis, and right leg.

Employees were moving a paving machine. The machine had been pulled forward, then put in reverse to back up a hill, when it revved up and went out of control. It had moved about 40 feet when one employee jumped out of it and was dragged under it. A second employee rode about another 40 feet, then jumped out to avoid a steep embankment. The first employee suffered a broken pelvis, and the second employee suffered a broken lower leg. Both were hospitalized.

An employee was operating a lawn mower. He fell off while making a turn and the mower ran over his right foot. The employee sustained a laceration on his right ankle.

An employee was driving a forklift. He fell off the machine during a turn, and one of its wheels broke his left foot. He was hospitalized.

An employee was traveling on a walker rider as it headed for the outer wall of the warehouse office. He jumped off the front of the rider which caused his legs to be caught between the base of the rider and the yellow safety rail. The employee sustained a fractured right fibula.

An employee was moving a large package from a slide when her left arm got caught between the slide (tilt tray) and the package. She was hospitalized with a fractured left arm.

An employee was operating a switch truck at a rail yard. He got out of the switch truck to unlock a container that was attached to a unit in front of him. The switch truck rolled and pinned the employee against the container. The employee sustained a fractured pelvis, a ruptured spleen, and a possible spinal cord injury.

An employee was sidestepping along the catwalk of a railcar when he lost his balance and fell approximately 4 feet to the ground. The employee's right hip was fractured.

An employee was working to realign the two top sections of a set of collapsible rollers (trailer-on-flatcar rollers). The rollers suddenly fell into place and pinched her left index fingertip against the lower sections. She suffered a partial amputation to the fingertip.

An employee was readjusting a safety hook to remove hardware from gang forms when his feet slipped. He fell to the ground, resulting in a broken left arm and wrist.

An employee was on a step ladder in a store aisle, making room on shelving for merchandise. She fell from the step ladder to the floor, suffering two fractures in her left leg. She was hospitalized, requiring surgery.

An employee was closing the side cargo door to a company van. The tip of his finger got caught between the door and the door jamb, resulting in a partial amputation.

An employee lost his footing while climbing an inclined ladder. He stumbled down one step and was able to stabilize himself by holding the railing and twisting his body, but he suffered an internal abdominal injury and was hospitalized.
