An employee was using an air pressure hose to drain a nitric acid line into a tote when the hose blew out of the tote and nitric acid burned the employee's legs and groin.

Two employees were working on the well head to bleed pressure off a tank so that ethylene glycol could be added. The sight glass on the tank exploded and struck one of the employees in the face, causing a severe facial laceration and requiring surgery.

An employee was tossing a damaged toilet into a compactor. The toilet bounced off the compactor and lacerated the employee's right wrist, cutting an artery and tendon.

An employee was operating a hydraulic jack to bend a frame when the jack came loose and struck the employee. They were hospitalized for a spleen injury.

A truck had been towed into the shop with a flat tire and a blown airbag. An employee checked the tire for damage and was airing the tire. The employee was installing the valve core when the bead slipped off the rim. Air escaped, and the aluminum wheel struck the employee's head and face. The employee became unresponsive and was bleeding.

On December 14, 2023, at 8:40 p.m., three employees were inspecting milling machines for startup. A few of the sleeves on the mill drum needed to be changed out. The injured employee was trying to input a sleeve on the mill drum using a hammer. The hammer hit the edge of the drum, which caused a metal piece to splinter off and strike the employee's cheek just below the left eye. The metal splinter embedded in the back of their eye socket, resulting in pain, swelling, and bleeding that required hospitalization.

An employee was pushing a line of carts into a vestibule when the door cable snapped and struck his right ankle. The employee suffered a fracture that required surgery and hospitalization.

Two employees were lifting a roll of steel poly bar (50 inches and approximately 150 pounds) from ground level to a height of 4 feet at the 66-inch rubber calendar. One employee dropped the roll, resulting in the poly bar contacting the injured employee's right knee. The employee was hospitalized with fractured right patella.

Employees were loading a 295-pound transformer onto a pickup truck using a forklift. The injured employee was acting as the spotter. The transformer hit the tailgate of the pickup truck and the spotter's right hand was struck by the transformer, resulting in amputation of the ring fingertip.

An employee was wearing full PPE while emptying a 55-gallon drum of phosphorus pentoxide and fainted, collapsing and striking their head against the ground. The employee was hospitalized and it is likely that heat caused by wearing full body PPE contributed to fainting.

An employee was troubleshooting a plug in the super-absorbent polymer line. The employee opened an inspection port and their right hand was caught in a rotary valve, resulting in the amputation of four fingers between the knuckle and the first joint.

An employee was conducting a quality control measure by taking a chemical sample for lab analysis. When the employee was opening the sample point, it broke off and crude 2-Ethylhexyl thioglycolate (EHTG) shot out at 265 degrees, and it covered the employee's legs and right arm and splashed the right side of their face. The steel caster hit a crack in the concrete, the weld holding the caster onto the cart broke, and the coil and cart tipped over onto the employee. He was hospitalized with a laceration on his forehead and a pelvic fracture on his right side.
