Final narrative
A beam was being loaded onto a flatbed truck. When the beam was being placed onto the flatbed, the wood and entire deck collapsed, causing the beam to shift. The concrete fell on the employee's left small toe; surgery had to be performed.
An employee was bent over using a multi-purpose saw to cut a knee-high plank lagging board that was jammed. During the cut, the saw blade pinched, causing the blade to kick back and lacerate the employee's right shoulder.
An employee fell approximately 15 feet through the floor of an enclosed scaffold structure while working on a bridge. The employee suffered a broken leg, wrist, and arm and was hospitalized.
A mixer truck driver was delivering a load of concrete to a jobsite. The driver was operating the controls outside of his truck when a 6x10-foot fascia wall gave way. One of the concrete fascia panels struck the driver's right calf resulting in amputation of the leg below the knee.
