Federal OSHA recorded a severe workplace injury at ETS LINDGREN L.P., 1016 WALDRON DR, DURANT, OKLAHOMA 74701 on — Amputations, affecting the fingertip(s).

An employee was cleaning a ribbon blender. He had the lid open, thus disengaging the interlock switch and disabling the rotating paddles. After he finished cleaning it, he closed the lid. The blender has a discharge hole at the bottom; unaware that the paddles in the blender were still rotating at a slow rate, the employee reached up through the discharge hole. The paddle amputated the tip of his index finger.

Hyponex Corporation

An employee parked a short tractor used to move trailers and then exited it. The tractor began rolling and the employee went to jump into the cab to stop the vehicle but his arm struck a metal bar on the cab and he fell, fracturing his left arm. The employee was hospitalized and required surgery.

Total Wall Systems, Inc.

Two employees were in the basket of an aerial lift. When attempting to bring the basket down, the basket got hung up on a perimeter guardrail post. The basket moved, causing one employee to strike their chest against the lift's guardrail. The employee sustained a fractured rib and was hospitalized.

Sunrise Transport Inc.

An employee was operating an electric pallet jack and moved it onto the lift gate. The lift gate was wet and slippery from running a previous pallet that contained product packed in ice. The jack starting sliding toward the curbside of the platform. As the pallet was sliding to the edge, the associate tried to keep the pallet level by placing his arm on the pallet, to keep it from falling. The pallet jack continued to slide and as the employee attempted to get out of the way, his finger slid across the pallet jack and was amputated.

JMC Steel Group

An employee was working on a pipe-threading machine, placing caps on the end of a pipe. The employee's finger caught the sharp edge of the pipe and was severely lacerated. The employee was hospitalized.

Albertville Quality Foods

An employee was removing boxes from a conveyor belt when the belt grabbed his gloved hand. The employee pulled his hand back and amputated his fingertip.

ETS-Lindgren, Inc.

An employee was performing maintenance and replacing a seal under a plexiglass viewing cylinder in a pre-expander machine. As the employee was testing the machine before putting it back into service, the viewing cylinder exploded and the employee sustained lacerations to their chest, face, right arm and hand.

Thermotron Industries

Two employees were replacing a compressor and were moving it when one employee dropped the compressor and it landed on his right hand on the floor. The employee sustained an amputation to his middle fingertip.

An employee was inspecting a sump at a gas station when a car struck them, resulting in a collapsed lung, fractured ribs, and abrasions.

Powercon Corporation

An employee was pulling a cover off a piece of electrical equipment when a flash occurred and they were electrocuted and sustained burns to their face and both hands.

An employee was walking into work when they tripped over uneven cement in the parking lot, and fell resulting in a hip fracture.

Rae Corporation

An employee was helping to move a 1,300-pound coil on a cart. The steel caster hit a crack in the concrete, the weld holding the caster onto the cart broke, and the coil and cart tipped over onto the employee. He was hospitalized with a laceration on his forehead and a pelvic fracture on his right side.

Meeco Sullivan

An employee was cutting a piece of metal rod with a metal cutting machine. His right middle finger became caught between the rod and the machine's table. The employee's fingertip was partially amputated.

An employee was working at her desk. She went to stand up and fell to the floor beside the desk. The employee sustained a hip displacement and required surgery.

Burgess Manufacturing of Oklahoma Inc.

An employee was processing wood boards at a chop saw when the saw malfunctioned and the blade cut her left hand and fingers. She was hospitalized and her little finger was surgically amputated.
