An employee was operating an electric pallet jack and moved it onto the lift gate. The lift gate was wet and slippery from running a previous pallet that contained product packed in ice. The jack starting sliding toward the curbside of the platform. As the pallet was sliding to the edge, the associate tried to keep the pallet level by placing his arm on the pallet, to keep it from falling. The pallet jack continued to slide and as the employee attempted to get out of the way, his finger slid across the pallet jack and was amputated.

Hyponex Corporation

An employee parked a short tractor used to move trailers and then exited it. The tractor began rolling and the employee went to jump into the cab to stop the vehicle but his arm struck a metal bar on the cab and he fell, fracturing his left arm. The employee was hospitalized and required surgery.

Total Wall Systems, Inc.

Two employees were in the basket of an aerial lift. When attempting to bring the basket down, the basket got hung up on a perimeter guardrail post. The basket moved, causing one employee to strike their chest against the lift's guardrail. The employee sustained a fractured rib and was hospitalized.

JMC Steel Group

An employee was working on a pipe-threading machine, placing caps on the end of a pipe. The employee's finger caught the sharp edge of the pipe and was severely lacerated. The employee was hospitalized.

ETS LINDGREN L.P.

An employee was cleaning a ribbon blender. He had the lid open, thus disengaging the interlock switch and disabling the rotating paddles. After he finished cleaning it, he closed the lid. The blender has a discharge hole at the bottom; unaware that the paddles in the blender were still rotating at a slow rate, the employee reached up through the discharge hole. The paddle amputated the tip of his index finger.

An employee was removing boxes from a conveyor belt when the belt grabbed his gloved hand. The employee pulled his hand back and amputated his fingertip.

A&S Services Group

An employee fell while exiting a trailer. The employee landed on their left side on the pavement, suffering multiple broken ribs on the left side. The employee was hospitalized.

Southeastern Freight Lines

An employee was hooking up a set of 28-foot trailers, attaching a converter dolly to the lead trailer's pintle hook. He backed up the lead trailer and parked it a foot from the converter dolly, then lifted the converter dolly and rolled it to the lead trailer to set it onto the hook. The converter dolly kicked up, and the employee's left hand was caught between it and the trailer. He suffered a severe laceration to the palm between the ring finger and middle finger.

An employee was cleaning the windshield of his truck. When he stepped down from the truck, he lost balance and reached to catch himself using the side mirror. The impact fractured his left wrist. The employee was hospitalized and required surgery.

HOGAN TRANSPORT

An employee was standing on the DOT bumper on the back of a standard 53-foot trailer. The employee lost his footing and his grip on the handle bar, and fell approximately 42 inches to the ground. The employee was hospitalized with a fractured left hip/femur.

ARMELLINI EXPRESS LINES

A driver was making a delivery. As he was climbing onto his trailer to get boxes, the strap he was using to pull himself up broke. He fell from the back of the trailer to the ground, landing on his right leg and breaking his femur.

An employee was cleaning leaves with a backpack blower when he fell from a ladder, resulting in injuries to his head, neck, and back.

An employee was operating a battery-powered broom to clean demolition debris off the top of the outermost bridge girder. The girder was approximately 11 feet above the demolition scaffold below. The employee fell approximately 8 feet to wind bracing below. The employee was hospitalized with a pelvis/tailbone fracture. Fall protection was in place at the time.

An employee slipped while descending in a stairwell, fell, and suffered multiple contusions/bruises to the head, neck, shoulders, knees, and back. She was hospitalized.
