Final narrative
An employee was loading material onto a rack in the bed of a pickup truck, and was in the process of tightening a ratchet strap to secure the load. As the employee was tightening the ratchet strap, the employee fell and struck his torso on the opposite side of the bed, breaking five ribs. The employee was hospitalized overnight.

An employee was cleaning up steel banding. The banding whipped and cut the employee, causing an inch-deep laceration about an inch above the right elbow.
An employee was retrieving PVC pipes from storage for a customer. The PVC pipe bundles were stacked. When the employee cut the bands on a PVC pipe bundle near the bottom of the stack, the bundles shifted and collapsed on top of the employee.
