An employee was feeding scrap metal into a chipper when his right hand was squeezed against a metal component of the machine. He suffered an amputation to the right index finger at the second joint.

On April 10, 2025, an employee was operating a brake press when two of its dies pinched her left ring and middle fingers. She suffered amputations to both fingertips, just above the fingernails. The machine was guarded with a light curtain at the time of the incident.

An employee was using a forklift to move a piece of angle iron. The angle iron slipped off the forklift and hit the employee's right foot resulting in a toe amputation.

An employee was manufacturing parts on a roll former machine. He went to remove the parts and his right hand became caught between the parts and the guard, resulting in partial amputation of his ring finger.

On March 23, 2023, at approximately 9:00 AM, an employee was operating an automated saw. As the employee went to adjust the coolant sprayers, the rotating blades caught his glove and pulled his hand in, resulting in three left-hand finger amputations.

An employee was operating a band saw when his right little fingertip was amputated.

An employee was operating a strip saw. As the employee shut off the saw, their right index and middle fingertips were amputated by the blade before it came to a complete stop.

An employee was using a table saw to cut a piece of wood for wall framing. The wood jumped, causing the employee's hand to move toward the saw, amputating their left index finger and causing a laceration on their hand.

An employee was operating a trim saw. While trying to free a jammed board she had just cut, she came into contact with the running saw blade, which amputated her right thumb.

An employee was using a shaper to cut wood when their right hand contacted the blade, resulting in a partial amputation to the right ring finger.

An employee was using a powered pallet jack. As he was maneuvering backward, his right foot slipped, causing the pallet jack to pin his right leg against a rack. He suffered a laceration between his ankle and his calf, as well as a fracture, and was hospitalized.

An employee was operating a high-density polyethylene (HDPE) fusion machine. The machine's carriage was directing a fitting that was being lifted from the machine when the employee's left ring finger was crushed in a pinch point, resulting in a fingertip amputation.

An employee was struck by a semi truck and pinned against a forklift while performing unloading activities. The employee was hospitalized with broken ribs and a torn left MCL.

An employee was on top of a railcar and was using a rod to push out some resin that had become lodged inside of the car. When he lifted the rod, it contacted a live, overhead wire and he was electrocuted. The employee sustained burns to 12-20 percent of their body.

An employee used a forklift to take a trash hopper to the dumpster. While driving back from the dumpster, the cord that had been used to open the hopper handle got caught under a wheel of the forklift causing the cord to tighten around the employee's left hand. The employee sustained amputation to the index, middle, and ring fingers.

An employee was traveling down the maintenance aisle of a warehouse on a single-pallet front rider jack. The jack malfunctioned, causing the brakes to apply; the employee fell forward onto the concrete warehouse floor. The employee suffered an injury to the left leg and was hospitalized, requiring surgery.
