An employee was using a powered pallet jack. As he was maneuvering backward, his right foot slipped, causing the pallet jack to pin his right leg against a rack. He suffered a laceration between his ankle and his calf, as well as a fracture, and was hospitalized.

An employee was driving a pallet jack down an aisle. Another pallet jack was about to strike his own, so he jumped off. His pallet jack then ran over his right ankle. He suffered a ruptured Achilles tendon that required surgery.

An employee was moving a scissor lift through a doorway. The employee was pinned between the scissor lift and the doorframe, sustained a back injury, and was hospitalized.

FLATIRON CONSTRUCTION CORP.

A concrete batch plant operator was assisting with clearing spoil piles using a skid steer. The skid steer backed into a stationary screen plant. The employee's left little finger was crushed between the controls of the skid steer and the screen plant, resulting in a fracture and laceration. The employee's finger was surgically amputated.

Cupertino Electric, Inc.

An employee was inspecting the overhead area of a single-person lift and repositioning the lift through a double doorway. As they were transitioning through the doorway, they were caught between the doorframe and the vertical mast of the lift. The employee was hospitalized with injuries to the nose, left maxillary sinus, left orbital bone, and left jaw, with fractures at the maxillary sinus area.

An employee was operating a roller and paving a small pathway next to a pavilion. The ground was on a slight pitch, causing him to reach up toward the roll cage to stabilize himself. His right fifth finger was pinched between the roll cage and the rafter of the pavilion. The employee sustained a partial degloving injury with partial traumatic amputation.

An employee was operating a stand-up forklift. The forklift drove into a rack in a warehouse, and the employee's right hand was caught between the rack and an upright supporting the forklift's overhead guard. The employee suffered a fracture and laceration to the hand, just below the little finger.

An employee was operating a high-density polyethylene (HDPE) fusion machine. The machine's carriage was directing a fitting that was being lifted from the machine when the employee's left ring finger was crushed in a pinch point, resulting in a fingertip amputation.

An employee was on top of a railcar and was using a rod to push out some resin that had become lodged inside of the car. When he lifted the rod, it contacted a live, overhead wire and he was electrocuted. The employee sustained burns to 12-20 percent of their body.

While driving back from the dumpster, the cord that had been used to open the hopper handle got caught under a wheel of the forklift causing the cord to tighten around the employee's left hand. The employee sustained amputation to the index, middle, and ring fingers.

An employee was walking to the processing area when she was struck by a forklift carrying stacked pallets. The employee fell to the floor. The forklift then reversed with the employee caught on the pallet, causing her to be dragged by the vehicle. She sustained a fractured left ankle.

An employee was helping a coworker transport a tall palm tree with a mini skid steer. The employee was severely shocked by a high-voltage electrical wire above the ground.

An employee was retrieving a Christmas tree from a shelf using a ladder. He missed a step and fell to the concrete floor. He sustained injury to his head and wrist.
