The employee in the office ran out and slipped on the spilled beans, falling onto the concrete floor and breaking her right ankle. She was hospitalized.

An employee was transferring chemicals from the tinder truck to the applicator when the transfer hose broke open and sprayed her face, especially her eyes, with 32% nitrogen, Acuron and Gramoxone SL. She was hospitalized.

An employee was filling an ammonia tank with ammonia when it spilled on him, causing dermal burns that required hospitalization.
