As he was attempting to patch the hole, he was caught under soy beans that spilled from the container and yelled for help. The employee in the office ran out and slipped on the spilled beans, falling onto the concrete floor and breaking her right ankle. She was hospitalized.

An employee was transferring chemicals from the tinder truck to the applicator when the transfer hose broke open and sprayed her face, especially her eyes, with 32% nitrogen, Acuron and Gramoxone SL. She was hospitalized.
