An employee was descending a portable extension ladder when he slipped and fell to the ground below, breaking bones. He was hospitalized and required surgery.
An employee was working in the office while a forklift operator was outside unloading a truck. The forklift operator struck a beam, cutting a hole in the container he was unloading. As he was attempting to patch the hole, he was caught under soy beans that spilled from the container and yelled for help. The employee in the office ran out and slipped on the spilled beans, falling onto the concrete floor and breaking her right ankle. She was hospitalized.
