An employee was monitoring a palletizer machine. The employee climbed up onto the machine to clear a jam at which time the machine completed its cycle. The employee suffered an injury to her right ankle and was admitted to the hospital for treatment and surgery.
An employee used an attached rope to pull a door down and close it, then placed his foot on the foot bar to lock it. The rope looped around his foot.
