An employee was buffing production parts and the rotating wheel contacted their left hand, resulting in hospitalization due to a laceration to the hand and a fracture to the hand and wrist.

An employee was changing a motor on an exhaust fan. The employee went to turn off the switch, located on the inside of the fan shroud when his left index finger was lacerated by the blade.

While walking toward the time clock, an employee tripped over a parts pan that was on the floor, injuring his knee.

An employee was removing a rack from a hanger. The rack dropped onto his right foot, causing an open fracture, as well as a puncture wound and contusions.

While stepping out of an electric car, an employee's right foot was caught between the accelerator pedal and the side of the car's frame, causing him to fall and twist/fracture his right ankle.

An employee was operating a machine when a wire pulled his right glove resulting in amputation of the index and middle fingertips.

An employee was grinding chicken when their right index fingertip was caught in the grinder and amputated.

An employee was sharpening peeler blades when their shirt was caught in the grinding wheel. The employee sustained tendon damage and a crushed right hand.

An employee was passing through a large metal door when the door closed on the employee, resulting in a cracked rib.

An employee was preparing to cut lumber on a panel saw when the clamp engaged and caught the employee's left middle finger, resulting in a fingertip amputation.

An employee was working on a portable stainless-steel vessel equipped with a butterfly valve at the bottom. The employee used a crane to raise the vessel and connected an airline to the valve control. Upon activating the compressed air supply, the valve opened. While holding the bottom of the vessel with their left hand, the airline disconnected and the valve closed on their left index fingertip. The employee sustained an avulsion of the fingernail, an open nondisplaced fracture of the distal phalanx, and partial amputation of the fingertip.

An employee was in training to learn how to change steel dies on a culvert machine. The employee was on top of the machine when the handheld control was activated, engaging the machine's auto-run function. The employee's feet were dragged into the roller dies, resulting in a left ankle fracture, a laceration between the ankle and knee, and amputation of the the big toe and second toe on the right foot.

An employee was walking up the stairs in an office area. Their right foot caught the edge of the stair at the top of the stairwell and they fell forward. The employee sustained a neck fracture.

An employee was operating a clipper machine and contacted maintenance when they noticed the machine was not working properly. Maintenance worked on the machine. While verifying that the machine was fixed, the employee's left index finger was amputated above the first knuckle.

An employee was feeding a strip of steel into a power press when it activated and contacted the injured employee's left hand. The employee suffered amputations of the first two digits of the hand.

An employee was helping a coworker transport a tall palm tree with a mini skid steer. The employee was severely shocked by a high-voltage electrical wire above the ground.

An employee was retrieving a Christmas tree from a shelf using a ladder. He missed a step and fell to the concrete floor. He sustained injury to his head and wrist.

An employee was inspecting a generator whose radiator was leaking. He slipped, and a fan blade in the generator amputated his thumb and index finger.

An employee was operating an agricultural tractor during sugarcane harvesting. The employee sustained a lumbar sprain due to vibration or motion from the tractor.

An employee was moving a 3-ton condensing unit, strapped down on a dolly, out of a garage. The strap broke, causing the employee to fall backward onto the brick pavered driveway. The employee suffered injury to a spinal ligament in the neck.
