Final narrative
In the research and development lab, an employee was testing a vacuum motor prototype. After installing the motor, he turned the power on to test it. The unsecured motor jumped out of its housing. When the employee grabbed the motor, his right index finger was amputated by the blades.

The injured employee was training another employee on how to operate a forklift and pick orders. The injured employee stepped between the forklift and a rack beam before the employee driving the forklift came to a complete stop. The employee driving the forklift turned to straighten out the forklift, at which time the injured employee became pinned between the forklift and the rack beam, suffering a broken bone in the shoulder area, broken ribs, and a facial cut.
An employee was operating a strip saw. As the employee shut off the saw, their right index and middle fingertips were amputated by the blade before it came to a complete stop.
An employee was using a table saw to cut a piece of wood for wall framing. The wood jumped, causing the employee's hand to move toward the saw, amputating their left index finger and causing a laceration on their hand.
