Parkdale Mills, Incorporated

A team was setting a vertical turbine pump for a chill water sump. To align the bolt holes, a forklift was used to bump the pump a small amount. The injured employee was checking if the holes were aligned when the pump was bumped again. Their right index finger was caught inside a bolt hole resulting in a fingertip amputation.

An employee was changing a saw blade when the saw machine activated. The employee's left ring and little fingers were amputated.

An employee was throwing blankets onto a blanket folding and stacking machine. A blanket got stuck in the machine and she attempted to remove it when the machine contacted her right little finger, resulting in a fingertip amputation.

Insituform Technologies, LLC

Two employees were working on a scissor lift approximately 15-20 feet above the ground, positioned beside a larger production line with a moving trolley. As the trolly retracted, its external motor struck the basket of the lift, causing the lift to tip over. The injured employee and the lift fell to the ground and they suffered a laceration to the right knee that required hospitalization.

An employee was working in a storage area consolidating pallets of yarn when his shoelace got caught on the corner of a pallet. The employee lost his balance and fell to the floor, resulting in a right femur fracture in the hip area.

An employee was helping to unclog a packaging machine when their fingers got caught in the machine, resulting in partial amputations to the index, middle, and ring fingers on the right hand. The machine was energized at the time.

On January 15, 2020, an employee was working at a welding machine and holding the tarp as it came out of the machine. As the machine began to cut the tarp, the employee attempted to cut the corner of the tarp with scissors. The machine's blade then contacted his finger, resulting in a partial amputation of the left fifth digit.

An employee was traveling down the maintenance aisle of a warehouse on a single-pallet front rider jack. The jack malfunctioned, causing the brakes to apply; the employee fell forward onto the concrete warehouse floor. The employee suffered an injury to the left leg and was hospitalized, requiring surgery.

Norflex, Inc.

An employee was operating a metal roller machine to roll a small piece of metal. The employee's right index finger became caught between the metal and the machine, and the fingertip was amputated.

An employee was shot in the leg with a pistol during an altercation with two people who had entered the store without permission.
