Final narrative
On or about August 20, 2015 at approximately 4:40 p.m., an employee received a hand injury while working on the pepper production line. The employee was feeling dizzy and went to sit on a small roller conveyor. As he sat down, he put his hand on a moving chain conveyor, which pulled his finger into the sprocket and partially amputated it.
On February 11, 2020, an employee was using a handheld circular saw to cut a plastic barrel when the saw slipped and struck the employee's left leg above the knee, causing a laceration that required hospitalization.
Employees were assembling and testing a butterfly valve unit and actuator combo. When the unit was function tested with air, the valve gate closed on an employee's left thumb resulting in a fracture and laceration.
