An employee was cleaning the deck and noticed a 10' piece of angle iron in the way. The employee attempted to move the angle iron when the left side of it became wedged between concrete and a metal support. The employee was standing next to an I-beam support structure. As the employee tried to move the angle iron back and forth, he slammed his right hand between the angle iron and the I-beam, pinching his right pinky finger and amputating the fingertip.

An employee was rearranging unit stacks inside a trailer. He stepped onto one unit (5.5 feet tall) to push the unit behind it. The employee fell between the two units onto the floor of the trailer, resulting in five broken ribs on the left side and a punctured left lung.

An employee was clearing a fault on her machine when she slipped on preprint corrugated sheets lying on the floor and fell to the concrete floor. She sustained a fractured right hip and a bruised right wrist.

An employee was operating a forklift and unloading a truck. After the truck was unloaded, the employee got off the forklift and proceeded to adjust the dock plate with a hook tool. The dock plate slipped and the employee went to adjust the plate with their hand when their left middle finger got caught. Their finger was amputated above the first joint.

An employee was rotating a carbide granulator blade when his finger was caught between the blade and a stationary part of the machine. The employee's right middle finger was partially amputated and they suffered an avulsion to the right ring fingertip.

As the employee was manually spinning the spindle, his index fingertip was crushed between the spindle's jaws and the grooving insert, resulting in a fingertip amputation.

An employee was walking down the off ramp from a box truck, carrying a door for delivery. A strong wind caught the door, causing the employee to lose balance and fall about 2 to 3 feet to the ground. He landed on his back and suffered a broken vertebra.

An employee was loading lumber from a rack into his truck by hand. He fell from the truck to the ground and sustained a fractured hip.

Hines Building Supply

An employee was delivering lumber to a customer's job site. He was using a truck-mounted forklift to transport the material to the staging area. The surface of the path of travel changed from asphalt to soil and rock with a slight downward grade. When the forklift moved onto the softer surface, it sank into the ground and its center of gravity shifted forward, causing it to tip over. The employee's right arm sustained a closed, supracondylar humerus fracture.

McCoy Corporation

An employee was helping two forklift operators move and inventory flat sheet metal that was staged on racks. The metal fell off one side and struck the employee's arms and abdomen. The employee sustained a laceration above the left eye, a left forearm injury, and a ruptured spleen. The employee was hospitalized and required surgery.

An employee was cleaning a press machine when their right ring finger became caught in the press, resulting in the amputation of the finger.

An employee was helping to lift the grating from a floor draining system when the grating slipped and landed on his hand, resulting in the amputation of his right middle finger at the first joint.

During a workshop meeting in a hotel, an employee heard a drilling noise, so he walked outside to see what it was. An explosion occurred (possible gas line) and his face, ear, and hair were burned. He also fell and sustained a pelvic fracture.
