An employee was adjusting block tooling equipment for an operation when his left ring finger was caught between the bolt and the tooling. The employee sustained an amputation and was hospitalized.

An employee was using a crane to lay a metal block on the floor. As he pulled the crane hook, his left little finger was caught between the block and the hook, resulting in an amputation that included the loss of a fingernail.

An employee was preparing the metal extrusion for stretching when the operator lifted it into position. The employee's finger became caught in the extrusion pinch point area, causing an amputation to the right fingertip.

An employee was operating a strip saw. As the employee shut off the saw, their right index and middle fingertips were amputated by the blade before it came to a complete stop.

An employee was using a table saw to cut a piece of wood for wall framing. The wood jumped, causing the employee's hand to move toward the saw, amputating their left index finger and causing a laceration on their hand.

An employee was positioning dunnage on a trailer while pipe was unloaded from a forklift. A pipe fell and the employee's right thumb and index finger were caught between the pipe and a piece of dunnage. The thumb was crushed, and the index fingertip was amputated.

An employee was using a lifting sling on a crane to move a straight-bar bundle and place it on top of an existing bundle. When moving the lifting sling, it became pinched between the bundle and a wood cleat. The bundle then fell onto the employee's left foot, resulting in crush injuries.

An employee was stacking metal tubes using a forklift. A stack became unstable and the employee got out of the forklift to steady it, instructing his helper to get into the forklift. The forklift movement caused some tubes to fall and strike the employee, resulting in an open fracture of his left hand.

Wilson Machine & Welding, Inc.

An employee was setting up a flexible gear coupling hub in a lathe, preparing to bore it out to fit a shaft. The hub assembly was not secured in the chuck when the crane support was removed, and the employee's right little finger was caught between the hub assembly and the inside of the lathe. The fingertip was amputated.

D & J MACHINERY, INC.

An employee was using a crowbar to loosen a flat bar when the crowbar struck the employee's right eye. The employee was hospitalized and required surgery.

GS II Building Products, Inc.

An employee was helping to lift the grating from a floor draining system when the grating slipped and landed on his hand, resulting in the amputation of his right middle finger at the first joint.

US Battery Manufacturing Company, Inc

A casting machine jammed. An employee's hand was caught in the machine, where a belt line caught and amputated the tip of his finger.
