Final narrative
An employee was threading a new roll of bags into the midstream bagger machine. The jaws of the machine include a heat seal and cutters. The machine activated and the jaws caught his finger resulting in the amputation of the left index fingertip.
An employee was inspecting trays as they came out of the descrambler machine. The employee went to reach for a damaged tray when his right index finger was caught between the tray and a sidepiece of the conveyor line, resulting in a fingertip amputation.
