On or about October 7, 2015, an employee of Alpha Controls & Services, LLC was performing a service call at a client location and stepped on an unsecured sewage injection pit access panel that gave way. His right leg fell through the 16" x 18" x 24" floor opening, resulting in a groin injury requiring hospitalization and surgery.

The Conlan Company

An employee was removing 65-foot concrete wall braces using a powered industrial truck and a choker sling. He was holding a tag line at the end of the brace. When the brace was removed the balance was uneven. The brace cantilevered and seesawed the employee 20 feet into the air. He struck the ground, fracturing his pelvis.

Ski Windham Operating Corp.

An employee was traveling down the right side of a ski run. At the bottom, there is a drop called the water bar. The employee hit the water bar, crashed, and tumbled to the bottom of the run, suffering a back injury.

Life School

An employee was at a school function at a trampoline park. He was doing flips when he landed on his back, injuring his neck. He required surgery and hospitalization.

Ring Energy Inc.

The belt on an air compressor had just been put back into place after turning upside down. An employee was working to take a picture of the identifying numbers on the belt so that a replacement could be ordered when the compressor rolled, causing the pulley to spin. The employee's right index finger was caught between the pulley and the belt, and the fingertip was amputated at the first knuckle.

Automatic Environmental System

An employee was receiving a 660-pound beverage vending machine for repair. While the machine was being moved from the back of the van to the lift gate, the caster wheel was caught between the van and the lift, causing the machine to fall onto the employee and pin him from the waist down. The employee was hospitalized with three pelvic fractures, a torn left anterior cruciate ligament (ACL), injury to a knee cap, and pain in the lower back/spine.

The Fletcher Group Automation, Inc

An employee was debugging/troubleshooting the teach sensor on a load-handling device (LHD) inside of a cell. The LHD unexpectedly moved to the home location and struck the employee on their head. The employee sustained a head injury, an aspiration-induced lung injury, and fractures to his lower back and right ankle.

Trane U.S. Inc.

The injured employee was holding a pipe while another employee was un-soldering a line. The line split apart and sprayed residual oil on the employee who was holding the pipe. The injured employee sustained second-degree burns to their left hand and arm.

Takeda Pharmaceuticals U.S.A., Inc.

An employee fell while moving a water canister, resulting in fractures to four fingers on her right hand.

Great Dane Ltd. Partnership

An employee was changing a die in a press when the die slipped and crushed the employee's left index finger. The employee sustained an open facture of the tuft of the left distal phalanx and a partial amputation.

Olympia Food Industries Inc.

An employee was mixing sauce using a sauce barrel mixer when his left little finger was amputated.

AbbVie Inc.

An employee was driving a boom lift (in the lowered position) in an exterior dock area. The lift s left wheels rolled onto base plates that covered a 3-foot-deep sump pit. The base plates failed, and one side of the lift dropped. The employee's left leg was caught under the lift basket, and he suffered a fracture to the lower leg including the ankle.

A temporary employee was carrying a nail gun through the warehouse when it hit his leg, activated, and fired a nail into the leg.
