On 11/14/15,at 1:30 p.m., an employee was driving a forklift while wearing a body harness and lanyard. The lanyard was run over, strangling the employee and breaking his right shoulder blade. He was hospitalized overnight.

National Gypsum

On or about November 28, 2016, at 12:25 AM, an employee was trying to dislodge a metal I-beam from a skid of wall boards. When he pulled to dislodge the I-beam, it separated from the wall boards and came down on his right hand, amputating his middle fingertip.

An employee was conducting routine maintenance on a mixer. New lid pins had been installed, and the employee was rotating by hand to ensure they had been installed correctly. The employee's left middle finger was caught between the lid and top of a pin. The momentum from the mixer continued, causing a crushing injury to the finger. The employee underwent a medical amputation from the top knuckle to the tip of the finger.

On July 29, 2025, an employee was placing dunnage spacers on the wall board near the production line. He began to feel lightheaded. The employee was hospitalized with heat-related illness.

The employee tripped over the formulation pallet and fell into a bucket of microbicide chemical. They sustained second-degree burns to the neck, back, right shoulder, left leg, and behind the right knee.

An employee was climbing a ladder to get on top of the breakroom. The ladder slipped, causing him to fall to the ground. The employee sustained fractures to his upper back, left foot, and ankle.
