An employee used pressurized air to help release a cylinder from its mold base. While the employee's finger was under the tool to feel for movement, the cylinder coupling came free and crushed his finger, resulting in amputation of the fingertip.

An employee was bitten on the leg by a spider while cleaning cobwebs and developed an infection.

An employee was operating a table saw when the material kicked back. His hand then contacted the sawblade, amputating his left index finger at the second digit and his middle finger at the first digit. His pinky was also crushed.

On December 22, 2016, at approximately 6:25 a.m., an employee was making adjustments to the GN machine 3030-7 contact heater when his hand slipped down into the cutting knife resulting in the amputation of the right index finger at the first knuckle.

An employee was operating a machine when a wire pulled his right glove resulting in amputation of the index and middle fingertips.

An employee was grinding chicken when their right index fingertip was caught in the grinder and amputated.

A subcontracted engineer was advising maintenance how to make a modification to the embossing roll system on a new line. Maintenance was bringing the motor and gearbox down with a crane. The load shifted when it was a few inches from the ground. The engineer went to catch/maneuver the gearbox and the fingers on his left hand were crushed between the I-beam base of the gearbox and the concrete floor. A finger was amputated.

On July 30, 2025, an employee was assisting co-workers in using a hoist to position an injection molding tool onto a work table. As the tool was being lowered, the slide section shifted, trapping and amputating the employee's right thumb tip.

An employee was manually lifting a film roll when their right middle fingertip was pinched between the roll and a metal beam, resulting in a fingertip amputation.

At about 10:00 a.m. on July 13, 2025, an employee was cleaning the adhesive from running rollers using a rag soaked in cleaning solvent to prevent contamination. The rag was caught between two rollers and pulled the employee's right hand between the rollers. The employee sustained right hand crushing injuries, de-gloving injuries (palmer and dorsal sides), and fractures. The employee was hospitalized and required surgery. The machine was not locked out/tagged out at the time.

An employee working on a sheet line was removing masking from the underside of plastic sheeting that was being lifted by a vacuum lift. The plastic sheet measured 4 feet wide, 14 feet long, and approximately 1 inch thick, and it weighed approximately 300 pounds. The vacuum lost suction and dropped the plastic sheet onto the employee's arm, pinning it between two sheets and resulting in a broken right forearm.

An employee was checking on a machine outside of the building and removed a cover to clean out a blocked area. His hand was caught by a rotary valve in a dust collection machine. The employee's fingertip was amputated.

An employee was working to clean a glue roller with a scraper tool. The tool and the employee's left thumb were pinched between two rollers, resulting in a partial amputation.

An employee was re-arranging boxes of pipe insulation on a pallet. One of the boxes fell from the pallet and knocked over an upright empty cylinder. The cylinder fell and crushed the tip of the employee's right toe. The employee's toe required surgical amputation.

An employee was securing the claw of a grapple truck to the truck bed. His left little finger was caught between the tie down strap and the rub rail of the truck, resulting in partial amputation of the finger.

An employee was cutting extruded aluminum framing material using a horizontal band saw. She went to grab a rag in the machine when the rag contacted the blade and pulled her hand in toward the blade, resulting in a partial amputation of the right index finger.
