An employee operating a truck was making a delivery on November 23, 2015. While exiting the vehicle, the employee stepped into a pothole, lost their balance, and struck their head against the pavement. The employee was hospitalized with a severe concussion and nausea.
An employee was pulling on a bale of corrugated cardboard boxes that were on a conveyor. The straps on the bale failed and the employee fell to the floor. The employee was hospitalized and required surgery for a fractured femur and/or hip.
